About the role

The Medical Director is our senior clinician. You'll lead the medical work that anchors our biggest sponsor-direct trials and runs our broader trial portfolio: encoding new trials, iterating screening rules with engineering, supervising our small medical contractor team, and being the medical voice with sponsors and customers. You'll also help shape how we develop our screening capabilities and how we approach hard clinical criteria as a company.

This is a hands-on role. You'll spend significant time hands-on with patient reviews and clinical reasoning, while also setting strategic medical direction and contributing to our biggest customer and business conversations.

What you'll be doing

  • Owning EMR review across our trial portfolio, managing a team of associates + hands-on with patient cases, surfacing patterns and feeding clinical signal back to engineering for rule refinement

  • Working with engineering to refine our automated EMR screening — defining the clinical logic for which patients to exclude outright, which need a human reviewer, and how to handle edge cases. You bring the medical reasoning and use AI to amplify it; engineering writes the code or contribute code yourself (we’ll provide training)

  • Encoding eligibility criteria for new sponsor-direct trials across our screening stack and reviewing the output of our AI encoding agent to iterate and improve

  • Strategic medical leadership across the company — pre-launch strategy on hard recruitment criteria, designing future-pool pathways for patients who don't qualify now but might later, partnering with product on how we evaluate hard criteria across trials, weighing in on new business and design conversations

  • Attending weekly customer meetings on our sponsor-direct trials — protocol amendments mid-flight, recruitment design, working through edge cases with sponsor medical leads

  • Driving improvements across our portfolio — documenting best practices, running funnel experiments and user research, turning what we learn from one trial into how we run the next

  • Partnering across the team — engineering, trial ops, leadership

What we want

  • Strong clinical training. You hold an MD, MBBS, DO, or MD-PhD. You can read a chart fluently and reason about clinical evidence under ambiguity. You don't need to be a specialist in any particular therapeutic area; we're hiring for how you think

  • Comfortable being hands-on. A significant portion of this role is doing the work-on with patient reviews and clinical reasoning. If you want a varied day that has you in-the-weeds but also requires high-level thinking, then this is for you.

  • A process and systems mindset. When you see work happening case by case, you ask "how do we turn this into a system?" and then build it. This is the muscle we care about most

  • Smart, proactive, resilient. You drive the work, raise what's broken, and stay steady when iteration is slow and edge cases keep coming

  • Quick learner across TAs. Our portfolio grows every quarter; you ramp fast on new conditions and new ways of working

  • Cohort and population thinking. You're comfortable reasoning about patient populations as a whole, not just one patient at a time — and you spot patterns across trials

  • High attention to detail. Precision in language matters and interpretation of I/E criteria need to be meticulously and logically mapped out

  • Strong communicator and team player. You build trust with engineering, sponsor MDs, the coordinator team, ops — and communicate clearly so others can do their work alongside yours. As the medical voice of the team, this includes explaining medical nuance to help build the product

  • Comfortable working remotely; we're a distributed team

  • Enthusiastic about picking up AI as a new, deep skill. We use agentic AI and vibe coding day to day. You don't need to be doing this already — you just need to be ready to learn quickly

  • Startup tolerance. Small team, ambiguous edges, figure-it-out culture

Bonus, not required: Experience working with EMR data systems. Prior clinical research, CRO, or pharma experience.

Location & compensation

U.S.-based, remote role with EST overlap required. We gather periodically in person.
